Abstract:
Background: Appendicitis is a condition characterized by an inflammation of the vermiform
appendix; it is one of the most common causes of the acute abdomen and one of the most
frequent indications for an emergent abdominal surgical procedure worldwide. The aim of this
study is to investigate gap on management outcome and associated factors of acute appendicitis
at Chiro General Hospital.
Objectives: The aim the study is to determine the management outcome and associated factors
of acute appendicitis in all surgical patients diagnosed for acute appendicitis and operated in
past four years at Chiro General Hospital, in Oromia, East Ethiopia from January 1/2018 to
August 1,2018G.C.
Methods: A retrospective cross sectional analysis was undertaken with complete survey on all
cases of acute appendicitis surgically managed at Chiro General Hospital from January 1, 2014
up to December 31, 2017 G.C. Descriptive analysis was conducted to management outcome and
associated factors of acute appendicitis and Odds ratio with 95% confidence level was estimated
using multiple logistic regression was carried out to analyze the association between the
independent and dependent variables. The level of statistical significant was declared at P-value
less or equal to 0.05.
Results:The most common types of incision performed were Lanz or Rock Davis incision which
accounts 335(92.3%). Intraoperative findings included inflamed appendix215(59.3%),
gangrenousappendix88(24.2%), perforated appendix43(11.8%), and appendiceal abscess
17(4.7%).The most common procedure performed was appendectomy 318(87.6%) followed by
both appendectomy and peritoneal lavage28(7.7%).The predominant postoperative complication
was postoperativeWound infection in 25 (6.7%) at 95%CI of [4.1,9.6]. Death of patients due to
appendicitis was not recorded during the study period.
Conclusion:Majority of the patients had favorable outcome but the other of them had
unfavorable outcome where they have improved but developed postoperative wound
infection/wound dehiscence. Age 20-49 years, residence of Chiro town dwellers and procedure
performed for non-complicated (inflamed/gangrenous) appendicitis were associated with
favorablemanagement outcome.
