Background: Uterine rupture is a serious life threatening obstetric complication during child
birth. The extent to which it happened and its management outcome is unknown in
ChiroGeneral Hospital (CGH).
Objective;The aim of the study was to determine management outcomes and factors associated
among women with uterine rupture inChiro General Hospital, in Oromia, East Ethiopiafrom
September 1, 2013 – Augest 30, 2017. Data was collected from July 15-20 /2018.
Method;A retrospective cross sectional analysis from September 1, 2013 – Augest 30, 2017
was undertaken with complete survey on all cases of uterine rupture during labor among
women who delivered in Chiro General Hospital. The calculated sample sizes 73, (p = 50%)
and using of correction formula.Since the calculated value was no great difference with data
present at the hospital seen during preliminary survey which is 90 cases. For this reason all
cases of uterine rupture were included in the sampling technique which was 90.Secondary data
collected using structured check list by trained data collectors. The collected data was checked
for its Completeness, entered, edited, cleaned , analyzed by bivariate & multivariate logistic
regression by using EPIDATA 3.1 and SPSS version 22.0.The data was described and
presented using narrative text, tables and graphs.
Result;-A total of 90 cases of uterine rupture and a total of 14,713 deliveries conducted in
Chiro General Hospital. Sixty seven(74.4 %) of cases in the age group of 20 - 35 years.The
most common causes were CPD74(82.2%), malpresentation and malposition 7(7.8%), previous
uterine scar 6(6.7%), 3(3.3%) of uterine rupture were due to augmentation and induction.
Sixteen (17.7 %) of maternal death and84 (93.3%) of neonatal death and patient were stayed in
hospital 1-20 days. In this study prolonged labor(AOR=8.996; 95%CI: 1.545, 52.378), mother
developed shock (AOR=9.92; 95%CI: 1.735, 56.707) and sepsis(AOR=7.02; 95%CI: 0.001,
0.14) had significant association with management outcome of uterine rupture.
Conclusion & Recommendation;-Uterine rupture was a serious obstetrics emergency caused by
prolonged neglected labor and it was catastrophic and preventable obstetrics
complications.Proper and timely labor followup to prevent prolonged labor and its
complication can reduce feto- maternal morbidity and mortality caused by uterine rupture. |
